For a public health researcher designing and interpreting antimicrobial stewardship trials, this study provides a rigorous methodological template for capturing indirect and total effects, showing that aggregate mortality reductions can mask within-population harm — a critical consideration for evidence-based prescribing policy in hospital settings and for trial designs evaluating interventions with transmission-mediated spillovers.
