A stochastic agent-based simulation demonstrates that two-stage randomized (2SR) trial designs can quantify spillover effects of antimicrobial treatment strategies that individually randomized trials systematically miss. In a modelled hospital ward with susceptible and resistant strains, shifting cluster allocation from 90/10 to 50/50 Drug A/Drug B reduced overall mortality by 4.00 percentage points, but this aggregate gain concealed a redistribution — resistant-strain deaths fell while susceptible-strain deaths rose, and among Drug A recipients mortality was 2.10 pp higher under the 90/10 strategy due to increased resistant-strain prevalence. For a public health researcher designing and interpreting antimicrobial stewardship trials, this study provides a rigorous methodological template for capturing indirect and total effects, showing that aggregate mortality reductions can mask within-population harm — a critical consideration for evidence-based prescribing policy in hospital settings and for trial designs evaluating interventions with transmission-mediated spillovers.
