In a post hoc analysis of the Phase III SPI2 trial, longitudinal serum neurofilament light chain (sNfL) was associated with real-world daily step counts from wearables in a 506-patient progressive MS cohort. Higher sNfL predicted fewer daily steps (IRR 0.97, p=0.014), and a 10% increase in step count corresponded to a 0.015 decrease in sNfL z-score (p<0.001); greater prior 6-month sNfL increases trended toward subsequent 6–12-month mobility decline (β=−141 steps per 1-SD increase, p=0.088). For diagnostic and translational strategy, this demonstrates that a blood-based proteomic marker can be anchored to objective, sensor-derived functional endpoints — strengthening the case for sNfL as a clinically actionable surveillance tool and for integrating wearables with fluid biomarkers in MS progression monitoring and trial design.
