This large-scale longitudinal study of 2,229 participants across the aging–Alzheimer’s disease spectrum demonstrates that MRI-visible enlarged perivascular spaces in the basal ganglia (PVS-BG) are early indicators of small vessel dysfunction and white matter hyperintensity pathogenesis. Researchers found that PVS-BG burden was associated with both greater white matter hyperintensity volume and faster progression, and covaried with higher amyloid-PET uptake cross-sectionally, though longitudinal amyloid associations were limited to amyloid-negative individuals. For a medical student focused on clinically relevant research, these findings highlight a readily available MRI biomarker that could identify patients at increased risk of progressive cerebral small vessel disease and potentially guide earlier interventions in neurodegenerative and cerebrovascular conditions.
