Inter-Rater Variability Challenges Glioblastoma Re-resection Decisions
Medicine · Neurology
A post-hoc analysis of the prospective, randomized DIRECTOR trial revealed substantial inter-rater variability among 11 surgical neuro-oncologists when assessing the resectability of recurrent IDH-wildtype glioblastoma based on MRI and clinical data. The study found that the proportion of cases classified as resectable ranged from 30 to 58 out of 69, with only slight agreement among raters (κ = 0.405). For a medical student focused on evidence-based practice, these findings highlight the subjective nature of surgical decision-making in neuro-oncology and underscore the need for standardized criteria to improve consistency in treatment planning and patient outcomes.
