RAAS Inhibition Reduces Bleeding in LVAD Patients: MOMENTUM-3 Analysis

This secondary analysis of the MOMENTUM-3 trial portfolio involving 1,855 patients with durable left ventricular assist devices (LVADs) found that renin–angiotensin–aldosterone system inhibition (RAASi) was associated with a significantly lower rate of hemocompatibility-related adverse events. Specifically, RAASi therapy at one month post-implantation reduced the two-year composite adverse event rate by 17% (aRR 0.83, p=0.0001), an effect driven predominantly by reductions in non-surgical bleeding and amplified in patients with sustained exposure (aRR 0.67, p<0.0001). For a medical student focused on clinically relevant research, this study highlights a practical, pharmacologic strategy to improve outcomes in a high-risk patient population, demonstrating how modulation of neurohormonal pathways can complement device advancements to directly enhance real-world patient care.
