A recent correspondence in The Lancet Diabetes & Endocrinology evaluates a new staging schema for the early diagnosis of dysglycaemia, following the 2024 International Diabetes Federation position statement endorsing the 1-hour 75g oral glucose tolerance test (OGTT) as an alternative to conventional 2-hour testing. The researchers propose that integrating the 1-hour glucose threshold of 8.6 mmol/L, termed “intermediate hyperglycaemia,” with HbA1c and traditional 2-hour OGTT criteria could enable earlier identification of diabetes risk. For a diabetologist deeply involved in clinical management and prevention, this refined, potentially more sensitive staging approach may allow for earlier intervention in high-risk populations in India and globally, improving the window for preventing progression to overt diabetes.
