A post hoc analysis of the Phase III SPI2 trial demonstrates that serum neurofilament light chain (sNfL) is bidirectionally associated with real-world daily step count in a large progressive multiple sclerosis cohort, with higher sNfL predicting reduced ambulatory activity and vice versa. Among 506 participants, a 10% increase in daily steps corresponded to a 0.015 decrease in sNfL z-score (95% CI −0.022 to −0.008; p < 0.001), and each 1-SD increase in sNfL was associated with 141 fewer subsequent steps (β = −141; 95% CI −302 to 21; p = 0.088).
