A new polysomnography study reveals that idiopathic hypersomnia (IH) is characterized by excessive stabilization of deep NREM sleep and reduced transitions toward wakefulness, challenging the traditional model that daytime sleepiness stems from poor sleep quality. Researchers analyzed overnight recordings from 62 IH patients and 81 matched controls and found that IH patients had fewer transitions from N3 sleep toward lighter stages, more stable REM periods, and reduced spindle amplitude despite preserved spindle density. This mechanistic insight reframes how sleep stability, not continuity, may drive hypersomnolence—offering a quantifiable physiological signature that could strengthen multimodal phenotyping and support future diagnostic or therapeutic targeting in sleep and neurodegenerative disorders.
