This multi-cohort study of 2,117 individuals establishes that baseline plasma p-tau217 thresholds achieve high and sustained accuracy (86–95%) for Aβ-PET positivity over up to 5 years of follow-up, demonstrating the diagnostic robustness of this blood-based proteomic biomarker. Longitudinally, 93–98% of p-tau217–positive individuals remained stable, while those in the intermediate zone progressed to positive at substantially higher rates (44–78%) than negative individuals (4–21%), with stable-positive and progress-to-positive profiles exhibiting accelerated tau accumulation and hippocampal atrophy. These findings support plasma p-tau217 monitoring as a clinically actionable, scalable tool for Alzheimer’s disease risk stratification and disease monitoring, directly advancing the integration of blood-based proteomic biomarkers into multimodal diagnostic workflows alongside imaging and clinical data.
