This study reveals that after ischemic stroke reperfusion, the mechanosensitive ion channel Piezo1 is hyperactivated in vascular endothelial cells by disturbed blood flow and tissue stiffening. Endothelial-specific Piezo1 activation triggers the release of cAMP-enriched microvesicles that are taken up by adjacent perivascular astrocytes, where the cAMP payload activates Epac1 and drives a wave of astrocyte apoptosis, exacerbating infarct volume. This previously unknown mechanochemical cascade identifies endothelial Piezo1 and astrocytic Epac1 as cell-type-specific therapeutic targets for preserving neurovascular unit integrity, offering a promising avenue for developing blood-based diagnostic assays that track endothelial injury and predict stroke reperfusion outcomes. The findings underscore the value of integrating mechanosensitive pathway biomarkers with multimodal data from imaging and clinical assessments to stratify patients at risk of ischemia-reperfusion injury.
