This study demonstrates that hormone replacement therapy (HRT) is associated with reduced odds of limbic-predominant age-related TDP-43 encephalopathy neuropathological change (LATE-NC), a common dementia mimic of Alzheimer’s disease, in older adults. In a cohort of 2,056 participants from the Religious Orders Study and Memory and Aging Project, researchers found that HRT use within 5 years before or after menopause (OR=0.70) or for 8 to 16 years (OR=0.44) was significantly associated with lower odds of LATE-NC at autopsy. These findings underscore the critical importance of HRT timing and duration as potential modifiable factors in dementia risk, providing a clinically actionable insight for therapeutic decision-making in postmenopausal women that may complement blood-based proteomic and imaging biomarkers in neurodegenerative disease assessment.
