Tau PET Underscores Medial Temporal Lobe Pathology in Alzheimer’s Disease

This clinicopathological study correlated in vivo 18F-flortaucipir (FTP) tau PET scans with whole-brain postmortem immunohistochemical (IHC) tau density maps in two Alzheimer’s disease cases at Braak stages IV and VI. Researchers found that FTP standardized uptake value ratios (SUVRs) showed a significant correlation only with AT8-positive tau in the more advanced Braak VI case. The results reveal that FTP PET systematically underestimates the severity of medial temporal lobe tau burden, a critical region for early disease progression, which may confound the evaluation of tau-targeted therapies.
